Behavioral Health Services
Specialized education and medication management is available for any patient demonstrating a need. Provides patient, family and caregivers with additional insight of acute and chronic mental health illnesses. -
Home Health Aide Services
Under the supervision of RNs, provide personal care such as bathing, shampooing and basic skin care. -
Home Infusion Therapy
Patients can receive IV medication and/or nutritional IV therapy in the comfort of his/her home. All education and teaching is provided by an RN. -
Nursing Services
Coordinate a patient’s plan of care and monitor their condition. Provide instruction and perform all other skilled nursing duties as required. An RN is on call 24 hours a day, seven days a week. -
Nutrition Services
Complete assessments on patients identified with nutritional requirements and/or risks associated with a diagnosis. -
Social Services
Assist patients and their caregivers in understanding the medical plan of treatment. Provide support and counseling as needed. -
Telehealth Monitoring Systems
Available for high-risk patients with a qualifying diagnosis. Upon discharge from home health care, patients receive additional teaching, coaching and monitoring for up to 90 days. -
Therapy Services (Physical, Occupational, and Speech)
- Physical therapists (PT): Provide therapeutic exercises that help restore movement, physical function and relieve pain in patients with impairments due to injury, disability or disease.
- Occupational therapists (OT): Provide therapy for engagement in meaningful activities of daily life such as self-care skills, work or social interaction.
- Speech therapists: Provide therapeutic treatment of impairments and disorders of speech, voice, language, communication and swallowing.

Experienced Professionals
Getting to the doctor’s office is not always easy when you have a medical condition. For patients who are recovering from illness or injury and who are confined to home, Southwest General offers home health services in Cuyahoga, Medina and Lorain counties. Our dedicated Home Health team brings individualized care to patients in the privacy and comfort of home. Services are available by physician referral for patients who meet the admission requirements. Visits are available Sunday–Saturday, 8 a.m.–4 p.m. Additionally, unscheduled visits can be arranged when medically necessary. For the safety of our patients, a registered nurse (RN) is on call 24 hours a day, seven days a week.
Our services are available not only to Southwest General patients but also to patients from other health care facilities. All services are provided under the medical direction of a patient's primary care physician.
